It is difficult to recognize the nail isthmus in the normal Nail, but it is easy to identify the region in nail disorders such as pterygium inversum unguis and ectopic Nail [1]. These include Inflammatory and noninflammatory onycholysis, paronychia, Chromonychia, Nail plate shedding, and pterygium inversum unguis as well as satellite reactions on the skin and mucous membranes [2]. We present a 50-year-old man who developed this abnormality on his right Fingers and toes after a stroke that resulted in paresis of his right side [3].
